Abstract

Cervical total disc replacement is a fast growing novel surgical procedure. The goal of our study was to evaluate the biomechanical behavior of various design of prosthesis after implantation within the c-spine, with a specific interest in the influence of the geometry of the implants. The 3D finite element modeling showed variations of the estimated kinematics, prosthesis stress and facets loads following changes in the center and radius of a ball-andsocket

arthroplasty. In spite of a great interindividual variability in vivo, the 2D analysis of dynamic flexion / extension radiographs of two series of patients with two different types of ball-an-socket prostheses demonstrated a propensity of the geometric center to influence the actual mean center of rotation. The development of the 3D EOS® imaging system for the cervical spine yielded us first to validate the use of this system then to study a double bearing prosthesis in the 3D space in vivo in a patient series. Ours results demonstrate the influence of the geometry of the bearing surfaces in articulated total disc prostheses at the cervical spine. The interrelationships between the prosthesis and the facets suggest the importance of patient specific modeling for guiding the surgical planning.
