Abstract
The trapeziometacarpal joint is routinely examined in clinical practice by hand surgeons. As a matter of fact, osteoarthritis of this joint is frequent providing pain and lower mobility of the thumb. However, kinematics assessment is very difficult because of the anatomical and biomechanical complexity of this joint. Therefore, we have developed and validated an in vivo protocol in order to assess kinematics of the trapeziometacarpal joint. We used an optoelectronic system to analyze the movements. The precision of this system and the reliability of this protocol have been studied. A custom software has been developed in order to compute the range of motion with successive rotations. We used helical axes in two principal movements to determine the position of rotation axes of the trapeziometacarpal joint. One hundred and one hands have been studied to obtain data-base. This proposed protocol allowed analyzing patient kinematics in hospital during consultations. Fourteen patients with osteoarthritis and nine patients who underwent trapezectomy have been analyzed.
This protocol will allow the assessment of osteoarthritis and treatment efficiency outcomes. Moreover, it could improve the trapeziometacarpal prosthesis design.
