Abstract

Idiopathic scoliosis is a pathology of multifactorial origin who leads to three-dimensional deformation of the spine. In most cases, it occurs during the growth phase of the preadolescent. Early identification of the patients who present a real risk of aggravation is necessary in order to prescribe an efficient therapeutic program. The aim of this thesis is to implement recently developed research tools (analysis of posture and mobilities, 3D geometrical reconstruction, finite elements modelisation) in order to determine their relevance in the evaluation of the risk of aggravation for the idiopathic scoliosis. A prospective multicentric clinical study allowed the observation of 72 scoliosis patients and their comparison to reference populations (asymptomatic, severe scoliosis), which lead to a resemblance classification based on 6 given parameters, descriptors of three-dimensional geometry. Complementary mechanical analyses based on finite elements models permitted, for some cases, to objectively evaluate the risk of aggravation. These tools bring to the clinical doctor new means of identification of the evolutive scoliosis, which allow for an early treatment, appropriated to the biomechanical condition of each patient.
